Marriage Records in Polk County Florida can be requested from the Florida Department of Health or any of its divisions such as Vital Records Office, Department of Revenue, and Insurance Information Institute. You can also request your marriage records online at the Florida Department of Public Health’s website, State vital statistics office, or any other designated location. The cost is generally based on the number of records requested and the retrieval time frame. These services are usually available at no charge to the individual.
To request these records you will need to fill out an application form and provide proof of your relationship with the person being recorded (the couple or the individuals). If you have a photocopy or original copy of your certificate (reproduced, certified or notarized), you may still request the information. Alternately, if you do not have a copy, you may choose to have one created and return it along with the application.
The information will then be stored electronically at the county record office for future use. It may take up to five days for the data to be uploaded into your computer system. The fee may also vary by county and includes turnaround time.
